Registered Nurse - Street to Home Service

About the role

Mater Health Services in partnership with Micah Projects has an exciting new opportunity for a Registered Nurse to work within the Homeless to Home Brisbane Common Ground Service.

This position will create pathways through a multi-disciplinary team approach with Micah Projects Street to Home Team to ensure social determinants of health are achieved for the individual. You will provide healthcare interventions for individuals experiencing multiple complex diagnoses within a community health setting.

Street to Home provides support to people who are experiencing chronic homelessness to move into and sustain long-term housing. These positions provide an exciting opportunity to become part of Inclusive Health – a partnership in care arrangement between Micah Projects, Mater Health Services and St Vincent’s Private Hospital Brisbane.

We will be offering this role on a part-time basis until 30 June 2019, with a view to extension pending further government funding. The successful candidate will be required to work alternate weekends (7.5 hours per week).

Salary range: $33.73 - $43.34 (Registered Nurse Level 1) per hour plus superannuation

About you

The successful candidate will have the following:

  • Current AHPRA registration to practice as a Registered Nurse (no restrictions)
  • Demonstrated post graduate nursing experience
  • Strong interest in Primary Health Care
  • Experience/interest in working with complex clients who have a history of addiction and or mental health issues
  • Ability to work autonomously with a high level of responsibility in a community setting
  • Ability to form and maintain strong collegial relationships across service organisations
  • Commitment to improving the health and services provided to people who are marginalized
  • Current open QLD drivers licence
