About the role
Maters Caner Care Centre is seeking a Nurse Navigator Cancer Care First Nations responsible for coordinating and navigating patient care in this maximum term 3-year, full time position.
This role involves supervising and coordinating the patient's cancer journey, with a focus on assisting Aboriginal & Torres Strait Islander individuals in connecting primary healthcare and Cancer Services. The Nurse Navigator will promote culturally capable and accessible care, collaborating with patients to navigate their healthcare and treatment pathways at the Mater Hospital. This includes addressing individual cultural, medical, and personal needs.
Additionally, the Nurse Navigator is the main point of contact for patients and coordinates support services to ensure a safe discharge. The role requires expert clinical expertise to conduct comprehensive holistic assessments and identify complex patients for optimal outcomes.
Salary range: $66.85 – $72.58 per hour plus 11.5% superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
The Nurse Navigator will develop culturally based assessment tools for Aboriginal & Torres Strait Islander patients to improve healthcare accessibility and outcomes.
- Conduct holistic assessments for patients starting cancer treatment
- Provide leadership and support to nursing colleagues
- Identify individual patient needs throughout cancer journey
- Act as a point of contact for patients
- Communicate effectively with patients and families
- Liaise with GPs and referral services
- Develop patient outcome measures and processes
- Contribute to governance and policy for cancer care
- Coordinate multidisciplinary care teams
- Create care plans for high-risk patients
About you
- Minimum of 5 years of clinical experience in Cancer Nursing
- Current Registered Nurse registration through APHRA and a current annual practicing certificate
- All mandatory education requirements must be maintained
Desirable
- Post Graduate studies in Cancer Nursing
- Desirable to have current chemotherapy competency
