About the role
The primary purpose of this position is to improve and maintain patient outcomes by providing support and education to patients with complex wound care needs and patients with stomas (new or existing), liaising with the clinical teams and families to provide nursing support and education to the patient, their family and the nursing teams.
The successful candidate will work 32 hours a week in collaboration and partnership with the CNC Wound Care and Stomal therapy to meet the needs of our patients as well as the organisation.
Salary range: $54.44 - $58.30 per hour plus 11.5% superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
- Be a clinical expert in the management of complex wounds and stomas, and in the prevention and treatment of Pressure Injuries.
- Support and contribute to the achievement of continuous quality improvement in the delivery of services to patients and their families
- Exercise proper courtesy, consideration and sensitivity in dealing with patients, members of the public and other employees
- Utilise high level written and verbal communication skills
- Maintain strict confidentiality of information pertaining to patients and other employees
- Participate in the systematic continuing development of your own work performance and skills
- Contribute to and participate in research projects
- Work in partnership with our clinical teams, our VMOs and other multi-disciplinary teams to provide the best outcomes for our patients.
About you
To be successful in the role, you will possess:
- Demonstrated clinical skills in expert knowledge in the management of the patient with complex wounds, stomas and in the management of pressure injuries.
- Demonstrated leadership qualities, problem solving skills and ability to work autonomously as well as in a team environment.
- Advanced communication and interpersonal skills within a multidisciplinary environment.
- The ability to participate in education and staff development, contribute strongly to the quality process and ultimately aim at improving patient care outcomes.
- The ability to plan and manage priorities and workloads.
- Proven ability to assess patients, formulate a plan of care and discuss best options in consultation with the CNC Wound Care and Stomaltherapy and the clinical teams.
- Ability to plan and manage wound care needs of the patient discharging from hospital with a wound or stoma.
