About the role
Mater Private Hospital Brisbane has an exciting opportunity for an experienced Registered Nurse to take an exciting step in their career as a Rehab Discharge Coordinator in a permanent full time role, 38 hours per week.
The Discharge Coordinator is a Registered Nurse who plays a crucial role by managing and coordinating the discharge process for patients. This role involves collaborating with healthcare professionals, patients, and their families to ensure a smooth transition from the hospital to post-acute care settings or home.
Salary range: $66.85 - $72.58 per hour plus superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
Conduct comprehensive quality patient assessments of patients' physical, emotional, and social needs to determine appropriate discharge plans
Identify and intervene proactively where circumstances contribute to an unsafe environment for patients/relatives or staff and escalate
Collaborate with healthcare providers to ensure seamless and coordinated care for patients
Support patients and their carers through education, resourcefulness, and provision of information to manage their conditions, make appropriate referrals and overcome barriers to accessing services
Collaborate with community resources, home health agencies, and rehabilitation facilities to ensure a seamless transition for patients
Manage the resources of the organisation to achieve both budget and service delivery targets
Actively manage the maintenance of safe work practices for the service
Participate in the development and implementation of policies, procedures, work instructions and guidelines
Ensure compliance with all regulatory requirements, including those of accrediting bodies and governing agencies
Facilitate and participate in internal and external audits and surveys
Maintain accurate and detailed records of the discharge planning process, including assessments, care plans, and coordination efforts
Conduct follow-up assessments to monitor the patient's progress post-discharge
Consult with the NUM on significant issues and any decision that falls outside of routine clinical and operational service delivery parameters
Provide educational opportunities and ongoing training for nursing staff
About you
To be successful in this role you will meet the following:
Current registration with the Nursing and Midwifery Board of Australia
Tertiary qualification in specialty (or working towards)
Minimum of 3 years full-time experience in specific clinical specialty
High-level interpersonal, written and verbal communication skills with the ability to engage with internal stakeholders at various levels
