About the role
Mater At Home is seeking an experienced and compassionate Clinical Nurse to join our team on a permanent part-time basis.
This is an opportunity to deliver high-quality, person-centred care in the community while supporting patients to remain independent at home and reducing avoidable hospital admissions.
You’ll play a key role in leading and supporting the SWIFT Model of Care, acting as a central point of contact across hospital teams, community referrers and emergency care partners to deliver seamless patient experiences.
Working autonomously and collaboratively, you’ll provide advanced clinical care, coordinate services across multiple providers and contribute to service growth, innovation and continuous improvement.
Salary range: $56.08 - $60.05 per hour plus superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
- Supporting and leading the SWIFT Model of Care portfolio within Mater at Home and acting as a key contact for internal and external referral pathways
- Undertaking comprehensive clinical assessments within home and care settings and developing individualised care plans that adapt to changing patient needs
- Coordinating person-centred care across hospital teams, GPs, specialists, allied health, community and aged care providers to support safe care at home
- Providing clinical leadership through mentoring, coaching and supporting nursing team members and students
- Driving quality improvement, risk management and service development initiatives across rapid access and hospital avoidance care models
- Delivering advanced nursing care and supporting optimal patient outcomes throughout the patient journey
- Maintaining accurate clinical documentation and ensuring safe, responsive and evidence-based care delivery
- Contributing to new treatment pathways and supporting service expansion opportunities across Mater at Home
About you
You are a confident and compassionate clinician who thrives in autonomous practice while building strong partnerships across multidisciplinary teams. You are passionate about delivering care differently and creating positive outcomes for patients outside the traditional hospital environment.
To be successful in this role, you will demonstrate:
- Current AHPRA registration as a Registered Nurse with no restrictions
- Current open driver’s licence
- Minimum 3 years’ experience as a Registered Nurse
- Previous experience as a Clinical Nurse, Liaison Officer or Navigator (highly regarded)
- Advanced clinical assessment skills and the ability to manage complex patient presentations
- Strong understanding of patient-centred and integrated models of care
- Excellent communication and stakeholder engagement skills with the ability to influence across internal and external teams
- Strong decision-making, negotiation and problem-solving capability
- Ability to work independently, manage competing priorities and adapt to changing patient needs
- Commitment to quality improvement, professional development and delivering exceptional patient experiences
Desirable:
- Postgraduate qualification in a related discipline
