About the role
The Mater Mothers’ Hospital has multiple exciting opportunities for experiences Nurses to be part of leading our Early Discharge Program – Hope Program . This is a permanent position that will see you working across a 7 day roster based out of our South Brisbane Campus and requires regular home visits to families post discharge.
The role of the HOPE Nurse/midwife is to provide education and support to families eligible for and enrolled in the HOPE (early discharge) program. This role is responsible for conducting assessments and home visits ensuring a smooth transition from hospital to the home. The primary purpose of the home visits is to weigh the infant, assess and support feeding progression, and provide support with ongoing NGT management. The HOPE Nurse will work closely with speech pathology and lactation consultants to ensure the delivery of comprehensive care to families in need.
Salary range: $54.44 - $58.30 per hour plus 12% superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
Identify potential patients for early discharge in NCCU and provide information and education to families
Potential to conduct telehealth consults/assessments with families enrolled in the H.O.P.E program
Conduct home visits post-discharge to weigh the infant, assess feeding progression, and provide support with NGT management
Utilise a structured approach developed in conjunction with speech pathology and lactation consultants to guide daily assessments
Offer basic advice on breast and bottle feeding, as well as guidance on NGT management
Identify when referrals are warranted for additional support from speech pathology or lactation consultants
Adapt and respond appropriately to parental concerns and follow relevant escalation processes as required
Empower parental confidence and competence in oral feed establishment through parent-friendly language and a hands-off approach
Maintain accurate and detailed documentation for patients on HOPE program
About you
Current registration with APHRA
Minimum of 3 years+ experience in Neonatal Critical Care
Neonatal qualification (graduate certificate or higher)
Proficiency in lactation and breastfeeding support
Skilled in telecommunication and digital technology
Strong interpersonal, written, and verbal communication skills
Ability to work autonomously and demonstrate good time management
Commitment to continuous professional development and best practices in healthcare
Ability to provide emotional support and reassurance to parents and caregivers
Capability to collaborate with the healthcare team and develop comprehensive care plans
Hold a current Queensland driver's license for home visits
