About the role

Mater Health has is seeking an experienced NDIS Coordinator to join our Allied Health team in this permanent, full-time opportunity based at South Brisbane.

The Allied Health Clinical Lead – NDIS provides authoritative guidance, consultation and clinical leadership to support coordinated care and safe discharge of NDIS participants with complex care needs across Mater Health hospitals. 

Working closely with multidisciplinary teams to guide, consult, advise and support clinicians, patients and services involved in the care and discharge of patients to ensure timely and safe transition to the community.  The position also supports building clinical capability through education, training and service improvement.

Salary range: $64.64 - $69.55 per hour plus superannuation and access to not-for-profit salary packaging up to $15,900  + access to many staff benefits! 

What you’ll be doing

Reporting to the Director of Allied Health, the purpose of the role is to deliver competent and professional assistance to clients, families, clinicians and care providers. Key tasks will include:

  • Provide clinical leadership, independent judgement and expertise to support the development of care pathways with internal and external stakeholders to achieve complex discharge planning and community transition for NDIS participants
  • Provide authoritative guidance, and consultation to support targeted solutions for NDIS patients at risk of prolonged hospital stays due to their complex care needs
  • Provide informed advice, advocacy and represent allied health services to ensure care planning is timely and duplication of services is avoided
  • Coordination of complex cases requiring escalation, advocacy and specialist advice in order to enhance patient outcomes and mitigate risk to the organisation
  • Develop and provide resources and support to health professionals to assist with navigating NDIS and enable access to information and education in a timely manner
  • Contributes to service improvement initiatives with the focus on reducing length of stay for people who are able to transition from hospital to the community

About you

The ideal candidate will have a degree/post-graduate qualification in an Allied Health profession from a recognised tertiary institution.

  • Eligible for registration as a health practitioner with the Australian Health Practitioner Regulation Agency (AHPRA) or eligibility for membership with the relevant health professional association (as appropriate)
  • Demonstrated high level clinical practice and independent clinical judgement in an acute hospital setting including formulating targeted and patient focussed plans with realistic and achievable goals based on contemporary practice
  • Demonstrated clinical leadership skills including the ability to negotiate with and influence a diverse range of stakeholders to facilitate complex discharge planning
  • Proven ability to communicate effectively, contribute to decision making and address conflict with profession and multidisciplinary teams, patients, carers and external stakeholders
  • Comprehensive knowledge of agencies, legislation, policies and resources relating to NDIS
  • Demonstrated ability to initiate, develop and evaluate service improvements to improve clinical services and patient outcomes
