About the role
Mater’s Hospital Brisbane has an exciting opportunity for a suitable qualified and dedicated Clinical Nurse to join our, Elder Care Team in the Emergency Department in a maximum term 3 month contract, part-time role 32 hours per week.
The Elder Care Team in the Emergency Department aim to see all frail older people presenting to the ED, including from residential aged care facilities (RACFs). The Clinical Nurse provides front line, specialised screening, assessment and care planning of frail older people, to ensure appropriate and coordinated care. They aim to streamline efficient ED processes leading to discharge home or to the ward in a timely manner, to improve health outcomes for older patients presenting to hospital.
Salary range: $54.44 - $58.30 per hour plus superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
Conduct comprehensive geriatric assessments for older adults in the ED.
Develop and implement individualised, goal focused, care plans in collaboration with physicians, allied health professionals, and family members.
Provide guidance on falls prevention, delirium management, medication safety, and discharge planning.
Support continuity of care by liaising with community services and primary care providers.
Completing referrals for assessments in the home through services such as My Aged Care
Work with ED physician and Allied Health to assess safety for discharge home or admission to hospital in a timely manner
Act as a clinical resource and advocate for geriatric patients within the ED.
Participate in quality improvement initiatives and staff education related to geriatric care.
About you
You will be driven by a challenging yet rewarding environment where you can bring your energy and experience to this dynamic ward. The successful candidate will have:
Current AHPRA nursing registration as a Registered Nurse (no restrictions)
Experience in emergency nursing or geriatric care preferred.
Strong assessment, communication, and clinical decision-making skills.
Commitment to patient-centered care and interdisciplinary teamwork.
Knowledge of geriatric syndromes, delirium, falls prevention, and chronic disease management.
Excellent written and verbal communication skills and the ability to liaise with a variety of different stakeholders both internally and externally at various levels
Demonstrated ability to operate both in a team environment and autonomously using initiative, sound reasoning and strong listening, negotiation and decision-making skills
Strong attention to detail and organisational skills
Commitment to ensuring the levels of service are delivered to patients at all time
