About the Role

Mater Private Hospital Bundaberg is seeking a highly skilled Clinical Nurse to join our team in a newly created Discharge Coordinator role. The successful applicant will secure Permanent Full Time employment working a 38-hour work week.

In this pivotal role, you will lead and coordinate the patient discharge process, ensuring safe, timely, and effective transitions from hospital to home or post-acute care settings.

You will work collaboratively within a multidisciplinary team to deliver person-centred care, optimise patient outcomes, and support continuity of care. This is an excellent opportunity for an experienced nurse who is passionate about care coordination, patient advocacy and service improvement.

Mater Bundaberg is a 59-bed hospital providing a broad range of surgical, medical and rehabilitation services. Our busy Perioperative Department includes three operating theatres and a procedure room, supporting over 4,400 procedures annually across specialties such as orthopaedics, general surgery, gynaecology and more. With dedicated Surgical and Medical/Rehabilitation wards, we pride ourselves on delivering high-quality, patient-centred care in a supportive and collaborative environment.

Salary range: $56.08 - $60.05 per hour plus superannuation andaccess to not-for-profit salary packaging up to $15,900  +access to many staff benefits!

What You’ll Be Doing

  • Conduct comprehensive patient assessments to develop individualised discharge plans
  • Coordinate and facilitate safe, efficient discharge processes across multidisciplinary teams
  • Collaborate with medical staff, nursing teams, allied health professionals and community services
  • Advocate for patients and their families, ensuring informed decision-making and consent
  • Provide education and support to patients and carers on post-discharge care and follow-up requirements
  • Liaise with external providers including community services, home health agencies and aged care facilities
  • Identify and manage risks, ensuring compliance with safety and quality standards
  • Monitor and evaluate patient outcomes post-discharge, addressing any concerns promptly
  • Maintain accurate documentation and contribute to continuous quality improvement initiatives
  • Support Nurse Managers in driving best practice, service improvements, and effective discharge planning strategies

About You

You are a compassionate and experienced Registered Nurse with strong clinical expertise and a passion for delivering high-quality patient care. You thrive in a collaborative environment and have the confidence to coordinate complex care needs.

To be successful, you will have:

  • Current APHRA registration with the Nursing and Midwifery Board of Australia
  • Post graduate qualification ( desirable not essential)
  • Minimum of three years’ experience in an acute clinical setting
  • Strong knowledge of the healthcare system, community services and discharge planning processes
  • Excellent communication, interpersonal and negotiation skills
  • Proven ability to work effectively within multidisciplinary teams
  • A proactive approach to problem-solving, risk management and service improvement
  • A commitment to ongoing professional development and patient-centred care
