About the role
Mater at Home is seeking an experienced and motivated Registered Nurse to join the team in a permanent full-time position.
This is a unique opportunity to deliver hospital-level care in the comfort and safety of patients’ homes, supporting people to receive acute and post-acute treatment while maintaining independence and avoiding unnecessary hospital stays.
As a Hospital in the Home (HITH) Registered Nurse, you’ll work autonomously while remaining closely connected to multidisciplinary teams across hospital and community settings. You’ll use your clinical expertise, judgement and problem-solving skills to coordinate care, respond to changing patient needs and deliver exceptional patient experiences.
Salary range: $44.06 - $56.52 per hour plus superannuation and access to not-for-profit salary packaging up to $15,900 + access to many staff benefits!
What you’ll be doing
- Delivering safe, evidence-based acute and post-acute nursing care within patients’ homes and community settings
- Conducting comprehensive patient assessments and responding to changing clinical needs with timely escalation where required
- Coordinating care with hospital teams, GPs, specialists, allied health and community providers to ensure seamless patient journeys
- Supporting patients and families through education, communication and shared decision-making
- Performing advanced clinical interventions and maintaining high standards of clinical documentation
- Contributing to quality improvement activities and supporting the ongoing growth and sustainability of the HITH service
- Collaborating with and supporting nursing colleagues while contributing to a positive and patient-centred team culture
- Maintaining safe practice standards, infection prevention and patient confidentiality at all times
About you
You are a confident and adaptable Registered Nurse who enjoys working autonomously while remaining connected to a broader multidisciplinary team. You are passionate about delivering innovative models of care and creating positive outcomes for patients outside of the traditional hospital environment.
To be successful in this role, you will demonstrate:
- Current AHPRA registration as a Registered Nurse with no restrictions
- Current open driver’s licence
- Minimum 3 years’ recent acute nursing experience
- Previous Hospital in the Home experience (highly regarded)
- Competency with Central line management, Venepuncture and cannulation (highly regarded)
- Excellent communication and stakeholder engagement skills with the ability to work across hospital and community environments
- Strong clinical judgement, decision-making and problem-solving capability
- Ability to prioritise, self-manage and adapt in a dynamic and autonomous environment
- Strong technology and information management skills
